Careful examination of the chest wall is essential, and abnormal heart sounds can tell you a great deal. Further testing is individualized. An exercise ECG is important in identifying the presence of ischemic heart disease and the amount of myocardium at risk. If the ECG is abnormal at rest, the patient should undergo a thallium stress test or exercise echocardiography. Are you dealing with a nagging discomfort in between your shoulder blade and your spine? No matter how many stretches you do, or how much time you spend foam rolling it, does it still persist? Pain in this area often gets labeled as a rhomboid issue because it is a relatively superficial muscle that you can appreciate from any anatomy chart. However, how to fix rhomboid pain sometimes may actually have little to do with the rhomboids. In this video, you'll learn the ultimate secret of how to fix rhomboid pain by taking a look at a very specific movement...thoracic rotation! Rhomboid pain is labeled as pain in between your shoulder blade and your spine (interscapular pain) that can even refer up to the base of your neck or down the middle of your back. It is often described as a nagging dull ache or pain, pressure, knifelike, pulling, or even a burning sensation (is that really rhomboid pain? Food for thought). You'll often find people dealing with discomfort in this area wanting to fidget around in efforts to feel better - moving their head and neck around trying to stretch, rounding their shoulders, squeezing their shoulder blades back, or twisting their upper back, you name it. The occasional soreness in this region is nothing to be concerned about, especially if you did a hard upper back workout or carried a backpack for a long time. However, dealing with constant discomfort or pressure in this area can become a real nuisance. Just because you're dealing with discomfort in between your shoulder blade and your spine doesn't mean we have to point the finger at the rhomboids every time! To fix rhomboid pain, we need to take a look at and consider all of the other anatomical structures in the area. If there was only one movement assessment I had to pick when it comes to evaluating interscapular pain, I'm going to look at thoracic rotation mobility. Poor thoracic rotation mobility can wreak havoc on the body and can definitely refer pain and discomfort to the rhomboid region. Poor thoracic rotation mobility is often coupled with poor scapular mobility/stability, which only contributes to the issue more! I have found that simply improving thoracic rotation mobility can improve interscapular discomfort. Counting down the four WORST positions to sleep in with a painful shoulder, and what you should be doing instead! Fix your sleeping position to minimize your shoulder pain and sleep better through the night. Simple tips from a physical therapist to help you get a more restful, pain-free night’s sleep and experience less shoulder pain when you wake up. The only thing worse than shoulder pain is shoulder pain that wakes you up in the middle of the night. We all know that sleep is important for healing; it’s the body’s time to recharge, recover, and repair. But when your sleep gets interrupted by the very thing it’s trying to heal… well… that’s a recipe for disaster. Most often the shoulder pain that you’re experiencing in the night or in the morning is due to your sleeping position with that shoulder. Most people unknowingly put too much pressure on it or hold it in such a position that stress is placed on already-impaired structures. If you suffer from shoulder pain that is waking you up at night or hurts so bad you can barely move it in the morning DO NOT despair! One of the best things you can do for it is to make some simple adjustments to HOW you’re sleeping to get a better night’s rest free from shoulder pain. In this video I’m sharing four simple tips to eliminate your nighttime shoulder pain. These are different fixes for your sleeping positions to decrease stress on your shoulder and help you to sleep better. I’ll present this in a “Top Four” format - we’ll countdown the four worst sleeping positions for your shoulder and I’ll give you some simple tips on what you can do instead to make for a more restful, comfortable night. WORST SLEEP POSITIONS FOR SHOULDER PAIN - AND WHAT TO DO INSTEAD! 4. ON YOUR BACK WITH YOUR HANDS BEHIND YOUR HEAD - it’s better to bring that arm down to your side, but even then it’s still in an extended position and can put stress on the front of your shoulder. Instead lay on back and supported under your your elbow 3. ON YOUR NON-PAINFUL SIDE, INJURED SIDE UP - Grab two pillows to hold it up (2 pillows better than one). Prop one under your elbow and the other in front of your body. 2. ON YOUR PAINFUL SIDE, INJURED SIDE DOWN - Best thing to do is unload it. Grab two pillows and put one about 6 inches below the other. This creates a canal to unload the shoulder as you have one supporting your torso and the other supporting your head/neck. 1. ON YOUR STOMACH WITH YOUR ARMS UNDER YOUR HEAD Honestly the fix here is just don’t do it. If you want to fix your shoulder pain, neck pain, upper back pain, and lower back pain - you need to stop sleeping on your stomach. I hope those tips help you to sleep better with your shoulder pain. I give those to my physical therapy patients regularly and I know they can be super effective. I hope you find them helpful in alleviating your pain as well. Round ligament pain is a discomfort that can be felt during pregnancy caused by a growing uterus and stretching ligaments that attach the uterus to different parts of the abdomen. You mentioned specifically that you've had some pain in the lower side of your abdomen, you're wondering what it is. It's really hard to say. The bottom line is if you have a persistent pain that is concerning to you or if you have cramping or bleeding it's always best to contact your doctor as soon as possible. One possible explanation especially for one sided pain is round ligament pain. Like I said, as the uterus grows the ligaments that attach the uterus to different parts of the abdomen cause it to stretch just like a rubber band or an elastic. It's usually associated with sudden changes in movement. Again, run it by your doctor and see what they have to think about it after checking you out and asking more specific questions. You mentioned that you're 38 weeks and you're having some lower abdominal pain, some lower back aches and it's just been killing you to be in certain positions. First of all, I want to express my sympathy to you because those last days or weeks of pregnancy can be a little torturous. Our bodies want the baby to come out but even though you're full term it is best until your baby stays in until about 39 weeks. That is why your doctor is saying that he won't induce you until you're 39 weeks. Studies have shown that babies do best at this point and their chance of going to the newborn ICU is lower, their chance of coming out and eating is higher, and having less complications is higher. All in all it's better if you can keep your baby in until 39 weeks. With all of that said the discomforts that you're describing can be terrible. If you just have generalized pelvic pain, that in most instances is due to the fact that your body is releasing a hormone called relaxin. That acts on the joints of your pelvis and causes them to soften and loosen, and that gives you that loosy-goosy feeling that makes you waddle and makes your pubic bone hurt. This is happening in efforts to increase the internal diameter of your pelvis. This increases the chance that your baby will be able to be born vaginally. Just tell yourself it's probably one of those things your body is doing in efforts to actually help you. Talk to your doctor about over the counter pain relievers that can help you until you end up delivering. You can also use a heating pad for 10 to 15 minutes on your lower back, or take a warm bath, that can oftentimes be very comforting. If you've been exercising up to this point in pregnancy continue to do so because it can actually reduce normal aches and pains of pregnancy. Try stretching out your lower back if it's bothering you especially before you go to bed. You may find that it helps you sleep a little more peacefully. Treat yourself to a massage, you're close enough that this may be your last opportunity you have to pamper yourself before you have your baby. Of course, ask your partner if they'll help rub your lower back if you have just some aches and pains at night. It's important for you to watch for signs that it's time to go into labor and delivery. Being 38 weeks and full term you can wait until your contractions are four to five minutes apart and that's been going on for at least an hour or two, as long as everything else is looking good. If you ever have vaginally bleeding, or you feel like your water is broken, like you're having a leaking of fluid that is very suspicious to you, or if you're not feeling your baby move as much as you usually do, specifically if you feel your baby move less than usual and you start paying close attention, lay down on your side and start counting movements. If you get ten or less in two hours, that in and of itself is a reason to go to labor and delivery. If you're contracting every ten minutes but you feel like your water broke, you go to the hospital. If you're contract every six minutes but you start bleeding or you're not feeling your baby move in between contractions you still go to the hospital. If everything else looks good, like I said, wait until your contractions are four to five minutes apart and increasing in intensity is frequency, and hopefully at that point your contractions are changing your cervix and you'll be in labor and can stay at the hospital and have your baby. Now, if you don't go into spontaneous labor in the next week then, like you mentioned, your doctor will induce you at 39 weeks so at least you have an ultimate due date to look forward to. The tailbone (or the coccyx) can become very painful if it’s injured or fractured. Here are 7 simple ways to help relieve coccyx or tailbone pain. Watch Dr. Jo show more ways to relieve tailbone pain: 🤍 For the first stretch for tailbone pain, you will start in quadruped or all fours. Open up your knees, but bring your feet slightly together. Lean back into a stretch, but not all the way to your feet. Then you can gently take your hips side to side. Next, staying in quadruped, you will do a cat/dog or cat/cow to stretch the hips and pelvis. Then you can go back into a full child’s pose or prayer stretch. The further down you go, the better stretch you will get. After that, you can lie all the way down on your stomach, and come up into a cobra stretch. If that is too much, you can start with a prone prop. For the last stretch for coccyx and tailbone pain, roll over onto your back, and do trunk rotations and bridges.
